Allow gift aid capture is missing from Donation Forms for databases with UK unlocked

Allow Gift Aid capture checkbox is missing on Donation Forms.  This checkbox should appear for databases that have UK unlocked.

We're currently evaluating this issue for a fix in a future patch or service pack.  

Steps to Duplicate

  1. Login to CRM
  2. Go to Help>About Blackbaud CRM
  3.  Notice UK is unlocked
  4. Login to BBIS
  5. Go to Site Explorer>Parts
  6. Add a new Donation Form Part
  7. Notice the Allow gift aid capture is not there
